Dependant’s Pass vaccination verification for foreign-born children

Dependant’s Pass vaccination verification is a pre-application document for a foreign-born child aged 12 or below who is applying for a new DP. The rule also covers a child whose earlier DP expired or was cancelled more than 30 days ago. The Communicable Diseases Agency issues the verification document, and the sponsor needs it before submitting the DP application.

Check the child’s age, birthplace and current or former pass status first. Then prepare clear vaccination records and allow time for the National Immunisation Registry process before setting a DP filing date.

Confirm whether the verification rule applies

MOM’s current Dependant’s Pass document guidance applies the requirement when all three facts are present: the child was born outside Singapore, is aged 12 or below and is applying for a new DP.

For this reason, a new DP includes a case where the earlier DP expired or was cancelled for more than 30 days. Existing DP, Long-Term Visit Pass and Student’s Pass holders do not need the document under the stated MOM rule.

Record the child’s date of birth, birthplace, present pass, expiry or cancellation date and intended application type. Do not infer the answer from a sibling’s case. Each child has a separate age and pass history.

Obtain the CDA document before the DP submission

MOM identifies the required item as the Verification of Vaccination Requirements for entry to Singapore document issued by CDA. The current sponsorship form states that the child must be issued this document before the DP application can be submitted and asks for the CDA reference number.

In practice, use the National Immunisation Registry portal linked by MOM for a new verification request. CDA’s vaccination-record contact page directs new foreign-born-child verification requests to that portal and provides an enquiry route for pending or submitted requests.

Keep the submitted request, reference number, uploaded-document list and issued verification document. The DP package should use the same child name, passport details and birth information.

Build the vaccination file from source records

Collect the child’s official vaccination record from the country or clinic where each dose was given. The record should identify the child, vaccine, dose and administration date. If several clinics or countries are involved, prepare one chronological index without altering the source documents.

At the same time, check passport names, former names, date of birth and document numbers. Add the birth certificate or legal guardianship record requested by the portal. Explain a genuine name difference with supporting evidence rather than editing the medical record.

The NIR guidance says non-English documents must be translated into English and both the original and translation uploaded. Files should be clear and use an accepted format and size. Review the live portal limits before submission because technical requirements can change.

Identify missing or uncertain doses early

Compare the available records with the vaccination requirements shown in the current CDA and NIR process. Do not guess whether a foreign vaccine name, combination product or date satisfies the requirement. Ask a doctor or CDA through the official enquiry channel where clarification is needed.

As a result, a parent may need to obtain a clinic letter, corrected record or additional vaccination. Allow time for clinical advice and the appropriate interval between doses. Immigration advisers should not make medical decisions.

Keep vaccination verification separate from the MOM medical examination

The CDA vaccination document and a medical examination or declaration requested in an In-Principle Approval are different steps. Vaccination verification is needed before the qualifying child’s new DP application. A later medical requirement depends on the individual IPA instructions.

Do not wait for the IPA to begin vaccination verification where the rule applies, because MOM requires the CDA document at application. Conversely, do not arrange an unrequested pass medical examination merely because the child needs vaccination verification.

Coordinate the application timetable

For example, put the NIR submission ahead of the intended DP filing date. Add time for missing records, translations, portal queries and any clinical follow-up. Do not book travel on the assumption that a pending vaccination request guarantees a DP outcome or date.

When the verification document arrives, check the child details and reference number. Combine it with the passport page, official birth certificate showing the parents’ names, English translations and other route-specific documents. Follow the current MOM upload instructions.

Protect the child’s health information

Vaccination records, birth certificates and passports contain sensitive personal data. Store them in the approved case system with access limited to the sponsor, authorised filer and relevant advisers. Do not send the file through public links or reuse it for another family member.

In addition, retain the issued verification and DP submission record according to the case policy. Remove unnecessary working copies after the secure record is complete.

Close the pre-filing check

Before submission, confirm that the rule applies, CDA verification is issued, the reference matches, identity records align, translations are attached and the sponsor has the remaining DP documents. A pending CDA request is not the required document.
